What to Bring to Your Neurology Appointment
To ensure a smooth and efficient visit, we ask that all patients arrive
30 minutes early and come fully prepared for their appointment at Neurological Specialties in Tampa. Bringing the right documents and medical records helps our neurologists provide the best possible care and ensures that your evaluation is as thorough as possible.
Checklist: Items to Bring to Your Appointment
- Photo ID (Driver’s license or government-issued identification)
- Insurance Card (For verification and billing purposes)
- Referral (if required by your insurance plan)
- List of Medications (Include all prescriptions, over-the-counter medications, and supplements)
- Medical Records & Test Results (Any relevant imaging, lab work, or previous neurology reports)
- List of Symptoms & Medical History (A written summary of symptoms, concerns, or recent changes in health)
- Emergency Contact Information
- Method of Payment (For co-pays or out-of-pocket expenses, if applicable)
Note: A 3.5% processing fee is applied to all credit/debit card transactions on which there is a Visa, MasterCard, or American Express logo (including HSA cards). Patient responsibility for co-pay, co-insurance, deductible, or other patient responsibility of out-of-pocket expenses or self-pay fees are due at the time of your appointment.
